I am a full arch implant surgeon in Phoenix, Arizona, focused on full mouth reconstruction, including the cases many surgeons refer out. Zygomatic, pterygoid, and palatal anchorage allow me to give patients who were told they don't have enough bone a fixed set of teeth.

"My journey began one Saturday morning while visiting my mother in Mesa Arizona, when I saw Dr. Miller on TV. I was surprised to see Dr Miller’s previous patients with the exact condition of my teeth. After being told that I was not a candidate for implants by my previous Dentist, I was determined to see if Dr Miller could help me. I went to a free consultation, they took X-rays, and explained the process for the procedure. Dr Miller reached out to let me know that I was indeed a candidate and explained that there would be no pain and no downtime but might be a little bruising, I was happy but still not sure if I wanted to go forward with the process because of pain.


Finally after a few months, I decided to go forward with the procedure. The best decision I’ve made! There was no pain, no downtime, a little bruising but well worth it! Finally getting around to posting the second case I did with @dr_paige_dds two weeks ago. PATZI protocol utilized to navigate this case. Very happy with the outcome. I learned a couple of great pearls at @fullarched course this weekend on things I can do to optimize the long term success of zygomatic implants.

Yesterday’s double arch. Pneumatized sinuses, impacted canines, and fairly soft bone. Right ptery couldn’t get torque even after numerous attempts and redirections. All implants got 50+ ncm of torque, and the zygos got about 80. I had to overextend the left zygo a bit to get the platform to the depth I wanted. I implantoplastied the left zygo apex as you can see in the radiographs. Overall very happy with the outcome. The patients first words when she saw her teeth were “I’m finally pretty”.

Atrophic case from yesterday. Very little bone in zone 1x, with just enough bone for two anterior implants. Cases was executed using PATZI protocol. Pterys got 60+ncm, anteriors got -45ncm, and zygo both for about 80ncm. Lower arch was a standard AO4. Super grateful to all of my mentors who have helped me get to where I am today and solve some of these more atrophic arches ??

Tag teamed a case with @vorholtdds in Santa Barbara Yesterday while I was in town with my family on vacation. I did the maxilla and followed PATZI protocol on upper arch courtesy of @danholzklau . Vorholt did the lower and had to squeeze some implants in on a super skinny mandible. All implants had great torque and patient had a full set of teeth for the first time in many years. Thanks for having me out Vorholt!

Fun upper arch with @4walldefect last week. Right pterygoid gave us zero torque even after multiple redirection attempts. All other implants torqued out great. Zygos were wrapped with some buccal fat that was tacked over the implants with osseous sutures.

Nice double arch yesterday morning! Patient has had broken down and failing dentition for years and finally decided to prioritize himself. Upper and lower arch completed in a little over two hours using PATZI protocol for the upper. The patients wife posted a pic of him and I after surgery in some facebook group and they are over the moon with the results. All I can see in the pic is that his bite is a little off since my lower fiducial marker moved during surgery ? All in all, very happy with this case.
